You must first achieve the Oracle Certified PL/SQL Associate certification. You must pass one additional exam: Exam #1Z1-141 Oracle9i Forms Developer: Build Internet Applications.

Continuing requirements:
Oracle will periodically announce requirements for recertification based on the release of new products and upgrades. You have 6 months after the announcement to recertify.
